Use this service if you have a positive pregnancy test, you can refer yourself to a midwife without the need to see a doctor.

You can use this service if you:

• are registered at the surgery

Once we have received your information we will contact you to book an appointment (the appointment lasts approx. 90 mins). Following the booking the midwife will email you direct with relevant information prior to your appointment.

• your first and last name, date of birth, sex, postcode, email and phone number
• if applicable, the details of the person you are completing the form on behalf of
