Via the internet and mobile phone including tablet apps, using your username and password provided by the practice.
How is consent given?
You apply for access please complete the online services form Register for online services or speak with one our care navigator team. The practice has the option to accept or deny patients’ requests to access their record. We only deny access in exceptional circumstances.
