Track a Referral

If you have been referred to somewhere other than The Leeds Teaching Hospitals Trust, ie, a community clinic or a private hospital, then once the surgery has processed your referral a text message will be sent to you asking you to call into anyone of the three surgeries.
Please come into the one that is most convenient for you to collect your referral paperwork.

You will then need to follow the instructions on that paperwork to activate and book your appointment.

So once again could we ask that you ensure we have up to date contact details for you. If you do not have a mobile phone number we will post the paperwork out to you.

Private Consultant

If you have chosen to be seen by a private consultant then please call into the surgery after 5 days to collect your referral letter.

We do not need to know where or who you are seeing, we leave the referral “open” and usually consultants are happy for you to take the referral letter with you to your appointment.

Private Test Referrals

If you are having a test done privately, ie, x-ray, ultrasound, MRI scan then we do need to know which private facility you want to be seen at as we need to send the referral direct to that unit where it is vetted by a consultant to ensure the request is appropriate, the private unit will then be in touch directly with you to arrange the appointment.
